Data Request and Use Agreement for Sharing Raw YRBS Data

Thank you for your interest in the youth of New Hampshire and our Youth Risk Behavior Survey. This document explains the process for sharing the raw data.

The YRBS Data Request and Use Agreement must be signed and returned to Mary Bubnis, HIV/Health Education Consultant, New Hampshire Department of Education, 101 Pleasant Street, Concord, NH 03301.

Upon receipt of the completed YRBS Data Request and Use Agreement, we will contact the contractor to acknowledge the sharing of the raw data for the project and send the data files requested.
