The UNH Institute on Disabilitiy presented Foundations in Transition, a five-part workshop series designed to provide:

• Training on how to improve the self-determiniation skills of youth with disabilities
• Tools to help young people develop a transition plan relevant to their areas of interest
• Resources for ensuring that young people are successful
