The role of the NH RESPONDS evaluation team is to implement the evaluation plan developed by the external evaluators with input from the Leadership Team. The evaluation plan outlines critical outputs, short term, intermediate and long term outcomes of the project and includes methodology and time lines for data collection and reporting. Responsibilities include assisting with instrument development and monitoring of data collection, analysis and reporting. Team members may be asked to disseminate key findings and recommendations to stakeholder groups (e.g, Advisory Board, State Board). The group meets every other month via teleconference call.

Create logic model and evaluation plan for all project goals, objectives and activities. Develop outcome and process measures. Conduct data collection, analysis and reporting functions.
