  • This form is for Child Nutrition Programs ONLY. If any updates to program permissions are needed, a new form needs to be filled out and submitted to elissa.l.mccullough@doe.nh.gov.  It is the responsibility of the program operator to ensure that the permissions are up to date.

NSLP Application Materials for SY 2024-2025

• The items below are required to be uploaded to the SY2024-2025 National School Lunch program (NSLP) and School Breakfast Program (SBP) application. If your SAU/RA operates other Child Nutrition (SN) programs, have a Food Service Management Company (FSMC), or are a Residential Child Care Institution (RCCI), additional documentation will also need to be attached. These documents can be found at the bottom of the page by program.
• Please note that the NSLP application needs to be fully approved prior to the first day of meal service for SY2024-2025 or claims for reimbursement can be affected.
